(CNN)-- Human error is to criticize in last week's gas-fueled explosion that tore through a strip club in the western Massachusetts city of Springfield, officials stated Sunday. The blast wounded a minimum of 21 people, including firefighters. An utility employee, responding to a report of a gas smell inside a structure, inadvertently pierced a gap in a high-pressure gas line at the foundation of that structure, according to a statement from the Office of the State Fire Marshal. Markings on the walkway incorrectly indicated where the line was. Once the pipeline was pierced, the employee called the gas business and the fire department to shut down the gas, and the location around the structure was evacuated. Investigators think gas from the leakage entered the structure and later on fired up.
